This page is for hospital providers working with Latrobe Health Services. The Australian Health Service Alliance (AHSA) manages our hospital contracts.
Which claim form do I need?
Download and submit the relevant form with every claim:
- National Private Patient Public Hospital Claim Form (PDF) for public hospital claims
- National Private Patient Hospital Claim Form (PDF) for private hospital claims.
All forms need signatures from the Latrobe Health member and an authorised hospital representative.
Where do I send claim questions?
Email the Latrobe Health Services hospital claims team at hcenquiry@lhs.com.au.
Where do I send Hospital Casemix Protocol (HCP) data?
Send Hospital Casemix Protocol (HCP) data directly to AHSA, not to Latrobe Health Services (sending it to us only adds a step).
Who do I contact about hospital contracts?
Hospital contract questions go to AHSA, not to Latrobe Health Services.
