Head to your nearest Shane Warne Legacy Health Check between 9 December 2024 and 23 January 2025, submit proof of your check, and earn two weeks free!* The free health checks are open to non-Corporate members aged 18 and over who have held a Latrobe Health membership for more than 30 days.

Our commitment to healthy hearts

As a not-for-profit private health insurer, Latrobe Health Services cares about healthy hearts and wants to make a real difference to our members and community. We've been delivering free health checks since the Gippsland Healthy Hearts Study in 2022 (read more here), and are proud to leverage this commitment to all Australians through our partnership with Shane Warne Legacy.

In August 2024, Latrobe Health Services became the Founding Partner of Shane Warne Legacy and announced a partnership expansion for another two years, as part of our annual community investment of $5 million. The goal is to deliver an additional 400,000 Shane Warne Legacy Health Checks, across Australia, on SiSU Health machines. 

 

What is a Shane Warne Legacy Health Check?

The health check stations check your risk factors for cardiovascular disease, using the following measurements;

  • Blood pressure
  • Heart rate
  • Body composition
  • Diabetes risk

Results will be provided immediately following the test, using the SiSU Health app. These can be taken to your GP for follow up, and will remain in the app for your reference.

Latrobe Health Services “SWL 2024 Promo"

Introduction
1. These Terms and Conditions apply to the Latrobe Health Services “SWL 2024 Promo” promotion (“Promotion”). You must comply with these Terms and Conditions and meet the criteria in clause 4 to be eligible for the Promotion.
2. Latrobe Health Services is offering a discount of two weeks free ("Reward") if you meet the eligibility criteria in clause 4 and 6 (each, an "Eligible Person").
3. This Promotion is offered by Latrobe Health Services Limited ABN 94 137 187 010 of 32 Darlimurla Avenue, Newborough VIC 3825. (“Promoter” or "Latrobe Health Services”).
4. This Promotion is only open to people who:
a) reside in Australia;
b)  be 18 years or over;
c) are an existing member of Latrobe Health Services who holds hospital, extras or combined cover, and have held membership for 30 days;
d)  have a valid email address and telephone number;
e) Are not a Latrobe Health Services member on a corporate discount; and
f)  are not a director, officer, management or employee of the Promoter or its related entities and must not have any immediate family members who are employees of the Promoter or its related entities. Immediate family means spouse, de facto spouse, child or stepchild (whether natural or by adoption), parent or stepparent, brother, sister, step brother or step sister.

5. The Promotion commences at 12:00am (AEST) on 9 December 2024 and closes at 11:59pm (AEST) on 23 January 2025 (“Promotion Period”).
6. To be eligible for the Reward, you must meet the eligibility criteria in clause 4, and:
a)  during the Promotion Period,
i. undertake a Shane Warne Legacy Health Check
ii. complete the ‘SWL 2024 Promo’ form and upload a screenshot of your validation email from SiSU Health Group of the completed test on Latrobe Health Services website: https://www.latrobehealth.com.au/two-weeks-free;
b) be an active member (no suspensions in place);
c) premium payments must be up to date at the time the two weeks’ free is applied.
7. Latrobe Health Services' decision is final and no correspondence will be entered into.
8. After an Eligible Person has met the criteria in clause 4 and 6, they will become entitled to have the equivalent of two weeks of their premium paid on their membership. Latrobe Health Services may take up to 30 days to process the extension, once it has been approved.
9. This promotion is redeemable once by one member per policy.


General terms
10. Nothing in these terms and conditions limits, excludes or modifies or purports to limit, exclude or modify the application of any provision, the exercise of any right or remedy, the imposition of any liability under the Australian Consumer Law, as well as any other implied warranties under the ASIC Act or similar consumer protection laws in the State and Territories of Australia, the exclusion of which would contravene that law or cause any term of this agreement to be void ("Non-Excludable Obligations"). Except for any liability that cannot be excluded by law, including the Non-Excludable Obligations, the Promoter excludes all liability, for any indirect or consequential loss or damage that you may suffer in connection with this Promotion or use of the Reward, including loss of opportunity, loss of profit, personal injury or property damage.
11. If the Reward is not available due to circumstances beyond Latrobe Health Services' reasonable control, Latrobe Health Services may substitute the Reward with another similar reward determined by Latrobe Health to be of equal or higher value.
12. To qualify, evidence of health checks must be submitted on the Latrobe Health Website while the offer is open.
13. The Promoter reserves the right to amend or withdraw the Promotion at any time.
14. Personal information is being collected by the Promoter for the purpose of conducting and promoting this Promotion. By joining this Promotion, an Eligible Person consents to storage and use of their personal information by the Promoter in accordance with its Privacy Policy. If the personal information is not provided, the Eligible Person may not participate in this Promotion. Our privacy policy, which is available at https://www.latrobehealth.com.au/privacy-policy/, includes our contact details, explains more about the types of personal information we usually collect and how we handle your personal information, as well as how you can seek access to and correction of your personal information, how to make a privacy complaint and how we deal with these complaints.
15. The laws of Victoria apply to this Promotion. Eligible Persons submit to the non-exclusive jurisdiction of the Courts of Victoria.
